New Patients

Dogwood Family Medicine welcomes new patients and families. Please click below for our new patient forms. Please complete the relevant Ages and Stages Questionnaire for pediatric patients if scheduled for Well Child Exam.  Please bring any available medical records, insurance card, ALL medications, a list of your concerns and completed paperwork (link below) to your first visit.

In order to allow adequate time for registration, please arrive 30 minutes prior to your scheduled appointment time. (Having your paperwork filled out in advance will make the registration much easier!)
