Effective June 1, Jackie Sitton, FNP is relocating her practice LifeWay Health. Jackie is available to provide care for you by contacting her at [email protected]. She has electronic copies of your medical records and can continue the care she has provided to you in the past.
The remaining five LifeWay Health providers are establishing a new practice named Dogwood Family Medicine in the same location that LifeWay Health is in now. A copy of your medical record will be kept at Dogwood Family Medicine should you wish to seek care from the providers there:
